I don’t think it means anything, but the four states with the highest sperm counts and the four with the lowest are all red states.
This, however, probably does mean something: It appears that sperm health is ameliorated by life in a big, empty state with little industry. The only problem is that there’s nobody there to have sex with.

DC is as blue as they come.
But not in the top four.
And also, of course, not a state.
But doesn’t fit the big empty state profile. It’s crowded and polluted.
An unpolluted environment is only one possible component in the equation. Diet is probably a factor. Weather may be important, since sperm are more active in colder temperatures. Genetics certainly would play a part.
Those other places with high sperm counts basically contain white people, D.C. has a concentration of black people. Does that lead to a conclusion? Not immediately or necessarily, but it suggests that the variable of race might need consideration. Certainly if we look at the places where sperm counts are high, we can easily solve the Sesame Street game of “One of these things is not like the others.” Given that DC has neither clean air nor cold weather, race seems like a possible explanation for its place on the list.
