I assume you are talking about nude body doubles, as opposed to cowboy trick riders and other stunt men from the early 1900s.
I don’t know which was the first, but Josephine McKim was the body double for Maureen O’Sullivan in a nude underwater scene from the 1934 MGM film Tarzan and His Mate.

That was some pretty serious underwater swimming so hard to say it was specifically for nudity. I think they would use a professional swimmer for that scene no matter what.
